This SMS Privacy Policy describes how PLX Digital Health, Inc. ("PLX Health," "we," "us," or "our") collects, uses, and protects your mobile phone number and related information in connection with our SMS text messaging program ("SMS Program"). This policy supplements our Privacy Policy.

We will only send you SMS messages if you have provided your explicit consent to receive them. You may opt in to our SMS Program by:

• Providing your mobile phone number and checking a consent box during account registration or at checkout
• Texting a designated keyword (such as START or JOIN) to our SMS short code or long code
• Submitting a consent form on our website or through a care enrollment process

Consent to receive SMS messages is not a condition of purchasing any product or service. You may use our services without opting in to SMS communications.

Depending on your preferences and enrollment, PLX Health may send the following types of SMS messages:

Message frequency varies based on your enrollment status, care program, and preferences.

In connection with the SMS Program, we may collect:

• Your mobile phone number
• Your mobile carrier and device type (collected automatically)
• Date and time of messages sent and received
• Message content when you reply to our messages
• Opt-in and opt-out records

We use information collected through the SMS Program to:

• Send the SMS messages you have requested or consented to receive
• Manage your SMS preferences and opt-in/opt-out status
• Comply with applicable laws and wireless carrier requirements
• Prevent fraud and unauthorized access
• Improve and analyze our SMS communications

We do not sell, rent, or share your mobile phone number or SMS opt-in data with third parties for their own marketing purposes.

We may share your information with:

• SMS service providers who send messages on our behalf, subject to confidentiality obligations
• Wireless carriers as necessary to deliver messages to your device
• Law enforcement or regulatory authorities when required by applicable law

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

• Reply STOP to any SMS message you receive from us. You will receive a single confirmation message and will be removed from our SMS Program.
• Log in to your account and update your communication preferences in your account settings.
• Contact us directly at support@plxhealth.com to request removal.

After opting out, you may re-enroll at any time by texting START or by updating your preferences in your account settings.

Please note: opting out of marketing SMS messages will not stop transactional messages related to active orders, prescriptions, or care unless you specifically request that all SMS communications be discontinued.

Message and data rates may apply. You are responsible for any fees charged by your mobile carrier for sending and receiving SMS messages. PLX Health does not charge separately for SMS messages, but your carrier's standard messaging rates apply.

Mobile carriers are not liable for delayed or undelivered messages. Delivery of SMS messages is subject to effective transmission from your network operator.

Our SMS Program is available through major U.S. wireless carriers including, but not limited to: AT&T, Verizon, T-Mobile, Sprint, U.S. Cellular, and others. Carrier support may vary.

We may update this SMS Privacy Policy from time to time. Changes will be posted on this page with an updated "Last updated" date. Continued participation in the SMS Program following any changes constitutes your acceptance of the updated policy.
